IVP DYE or Latex Allergies
Our office does not support a latex free environment. If you have one of these allergies, please call our office and let one of our staff know prior to your appointment.
Prescriptions
Prescriptions will be filled every business day before 5:00pm. We do not refill prescriptions immediately unless it is an emergency. Please have your pharmacy call them in. We do not do any refills on weekends or holidays.
Motor Vehicle Accidents
If you are coming to see Dr. Wheeler after a motor vehicle accident on your health insurance. We must have one of the following.
- Either a subrogation letter from your third party (Motor Vehicle) coverage insurance.
- Lien filed from your health insurance company.
Please have one of the two items with you at your appointment.
HMO’s
If your primary physician has referred you under your HMO, you will need to bring a copy of that referral with you, or make sure we have one here from your Primary Care Physician.
Films
In order for us to determine your need for surgery, you must bring the actual MRI or CAT scan films with you to your appointment. Please ask the receptionist where your films are being picked up, if the report was put in the film jacket.
We cannot view a disc or cd, we must have the actual films.

Provided below are various forms that require completion prior to your visit and/or surgery. Please download the appropriate form as directed by your physician and/or staff by clicking on the name of the form. If you can not access the form, please click on the Adobe box.
